Community Health Worker Apprenticeship Program

Community Health Workers (CHW) work directly with individuals and families to help them improve their health and access to services they need. As trusted members of the community, CHWs connect people to healthcare, social services, and community resources such as housing, food assistance, transportation, and behavioral health support. They provide education, guidance, advocacy, and personalized support, helping people navigate complex systems, overcome barriers, and make informed decisions about their health and well-being. CHWs play an important role in improving health outcomes and strengthening healthier communities with a wide variety of populations.

Who Can Be a Community Health Worker?

Minimum Qualifications Include:

  • Applicants must be at least 18 years of age and have a high school diploma, GED, or equivalent credential. Please submit official high school and any post-secondary transcripts. If you earned a GED or other equivalency credential, include a copy of your certificate. 
  • Veterans who would like military training or experience considered should submit a DD-214 or equivalent documentation.
  • Specific employers may have additional requirements such as, but not limited to: drug test, background check, physical examination, or bilingual skills.

  • Do you want to become an

    Applications Developer?

    Develop, create, and modify general computer applications software or specialized utility programs. Analyze user needs and develop software solutions. Design software or customize software for client use with the aim of optimizing operational efficiency.

  • Do you want to become a

    Plumber?

    Assemble, install, alter, and repair pipelines or pipe systems that carry water, steam, air, or other liquids or gases. May install heating and cooling equipment and mechanical control systems.

  • Do you want to become an

    Ironworker?

    Raise, place, and unite iron or steel girders, columns, and other structural members to form completed structures or structural frameworks. May erect metal storage tanks and assemble prefabricated metal buildings.

  • Do you want to become an

    Electrician?

    Perform preliminary work with the material and tools of the trade; residential and commercial rough wiring and finish work including the installation of various kinds of wires, cables, and conduits per the National Electrical Code; industrial lighting and service installation; motors controls and installation including emergency generators, relays, and timing devices.
